All "affected" and "authorized" personnel should be trained on the use of these lockout/tag-out devices. These devices include, but aren't only restricted to locks, chains and padlocks. Each device is uniquely identified and labeled, including the date and time of the application and the name of the person who is authorized to use it and the reason for the lockout or tag-out (repair, maintenance, etc). It must be clear that these devices are not able to be removed until a thorough LOTO procedure has been followed.
