A How-To Guide For Private ADHD Titration From Beginning To End
I Am Psychiatry

There is currently a nationwide shortage of medication to treat ADHD symptoms. This is due to manufacturing issues and the growing demand for ADHD medications worldwide.

Our service can offer private ADHD testing as an alternative to waiting for NHS funding through Right to Choose. Our assessments cost include the cost of one prescription.

Titration

A comprehensive assessment of your ADHD symptoms will be conducted by an experienced consultant Psychiatrist. This includes a thorough medical history, as well as an evaluation using the typical ADHD questionnaires and rating scales.

After your assessment You will be advised that medication may be beneficial for your situation. This will happen at your follow-up appointment (which can be scheduled in person by video link, in person or via phone). Your clinician will discuss the different options for medication available. If you decide to begin medication, we will help you the opportunity to increase your dosage to ensure the best balance of benefits and side effects for you.

This is a process which can take several weeks. It involves adjusting your child's medication gradually until the proper balance is reached. It is not uncommon to to adjust up to three different medications before you find the one that best fits your child's ADHD. It is essential that parents take part in the titration and keep an eye out for their child's behavior so they can let the doctors know whether the medication is working or causing adverse reactions.

Unless you have decided to return to NHS treatment, your medication will be issued with a prescription from a private pharmacy that you'll need to pay for at the pharmacy (this is not included in your consultation fees). You'll be required to attend every six months to check your medication - this includes blood pressure and pulse readings. Annual reviews are also required.

Follow-up appointments

Once the assessment is completed and you are satisfied, you can schedule appointment with the psychiatrist. Typically, the appointment will take place via a zoom video call, although in certain cases it is possible to do an audio only consultation (again at an extra cost).

At this point we will discuss treatment options and decide on a custom-made ADHD medication titration program. This will include a discussion and review of your dosage of medication and any side effects you may be experiencing. The cost of these titration consultations is PS150 for face-to face sessions and PS60 for online or phone video consultations. The pharmacy will be charged for any medication costs.

The psychiatrist will ask questions regarding your family history, the impact of your ADHD symptoms on your daily life and any relationship problems or mental health issues that you might be experiencing. This is because ADHD can be a genetic disorder and it is crucial that we assess any family history that might suggest that you are at risk of developing the disorder.

It is important to be honest and truthful to the doctor during the assessment, since the information they receive will be used to determine whether your ADHD is in line with the diagnostic criteria. This is a crucial step, as a wrong diagnosis could lead to inappropriate medication prescription.

If you decide to be treated for ADHD we'll schedule a follow-up appointment with a psychiatrist who specializes in this field. This can be scheduled within the first few days of your diagnosis if you want to start treatment immediately, or within two weeks if would like more time to think about the implications of your diagnosis and the possibilities available to you.

We suggest that if you are unsure about your preferences, talk to your GP prior to scheduling your private appointment. Certain providers have different policies on whether or not they require an GP letter. We also suggest that you check whether your GP accepts a 'Shared Care Agreement which means that once discharged from the clinic you will be sent back to them to get the prescription for the medication.

Medication

Medication is a key aspect of the treatment process for many people who are seeking a private ADHD diagnosis. It isn't easy to obtain an appointment with a psychiatrist on the NHS. NHS waiting lists are notoriously long, and GPs can be reluctant to refer patients on to specialist services.

One way to get around this is to request your GP to refer you through Right to Choose (see psychiatry-uk and ADHD 360). This means that you can select a provider that offers a private assessment and titration. This can reduce your wait time considerably and can be more convenient when you are working and have a full schedule.

Once you've been diagnosed, your doctor will examine you to determine if require medication adjustment. You'll have to fill out forms that are different for each provider but the most frequent ones are self-reporting forms and medical histories dating to your childhood and into adulthood. These will help your clinician identify any conditions that could indicate that you shouldn't be taking ADHD medications if they are prescribed.

If you are being titrated the medication will be a combination stimulants and nonstimulant medicines. They will be examined and monitored to determine the best combination for you. The titration process can be lengthy and may need several additional appointments before you are stable on your dosage.

The frequency of your follow-up appointments will be contingent on the type and amount of medication you're taking. They could be scheduled monthly or quarterly. Your physician will make sure that you are stable with the medication, note any side effects you've experienced and make any necessary adjustments to ensure that you remain on a safe dosage. You will be required to go to an annual health check-up and routine health examination which includes blood pressure, pulse, and weight measurements.

Some people might be satisfied with their medication and don't need to discuss their titration process further. For those who require further discussions regarding their titration and treatment plans, it's possible that they will be passed back to the same private clinic that performed their initial assessment. In this scenario the titration will be included in the consultation cost. The medication will then be provided through an individual prescription. This can cost more than an NHS prescription as the pharmacy will charge for the medication.
