Private ADHD Titration

There is a current national shortage of medications that treat ADHD symptoms. This is due to manufacturing issues and the growing demand for ADHD medications worldwide.

Our service can offer private ADHD titration as an alternative to waiting for NHS funding through Right to Choose. Our assessment fees cover the cost of one prescription.


Titration

A thorough assessment of your ADHD symptoms will be conducted with a highly experienced consultant psychiatrist. This includes a thorough medical history, and an evaluation using the common ADHD questionnaires and rating scales.

Following your assessment you will be advised if medication is considered to be beneficial in your case. This will be discussed at your follow-up appointment (which can be scheduled in person, via video link or by telephone). Your physician will explain to you what medication options are available and, if you decide to begin taking medication, we will offer to titrate the dose to maximize the benefits/side effects balance for you.

I Am Psychiatry could take a few weeks and it involves gradually altering your child’s medication until the right balance is achieved. It's not uncommon to need to titrate three or four medications before finding the best one for your child's ADHD symptoms. It is important that parents participate in the titration process and be on the lookout for their child's behavior so they can inform doctors whether the medication is working or causing adverse reactions.

If you decide to return to NHS care, your medication will be issued with a private prescription which you will need to pay for at the pharmacy (this is not included in your consultation fee). You will need to attend six-monthly appointments to check your medication. This includes the measurement of pulse and blood pressure. Annual reviews are also required.

Follow-up appointments

After the assessment is completed, you will be able to schedule follow-up appointments with your psychiatrist, if appropriate. The appointment is usually done through a zoom video call. However, in some instances, it could be possible for an audio-only appointment to be conducted (again with an additional cost).

In this session, we'll discuss any treatment options and agree upon a bespoke individualised ADHD medication titration strategy. This will include an assessment of your medication dosage and a discussion around any negative side effects you may be experiencing. The cost of these titration consultations is PS150 for face-to face sessions and PS60 for online or phone video consultations. The pharmacy will charge you for any medication costs.

The doctor will also ask you about your family history and the impact your ADHD symptoms can have on your life, including any relationship issues you might be facing or any other mental health issues you may be suffering from. ADHD is a genetic condition, which is why it's crucial to examine your family background to determine if you are at risk of developing this disorder.

During this assessment it is essential that you are truthful and honest with your doctor in the sense that they will use the information you provide to determine if you meet the diagnostic criteria for ADHD. This is a crucial step, since a wrong diagnosis could result in an inappropriate prescription of medication.

If you've decided you'd like to receive treatment for your ADHD, we will then set up a follow-up appointment with one of our psychiatrists who specialize in this area. This can be scheduled within the first few days of your diagnosis if you want to start treatment right away, or within two weeks if want to consider the implications of your diagnosis and the possibilities that are available to you.

If you're not sure which one you prefer, we suggest that you speak with your GP prior to scheduling your private assessment as some providers have different rules regarding whether they require the submission of a GP referral letter or not. Find out if your GP accepts a Shared Care Agreement, which means you will be referred to them when you leave the clinic for prescription medication.

Medication

Medication is an integral element of the process for many people who are looking for an official ADHD diagnosis. It isn't always easy to obtain an appointment with a psychiatrist on the NHS. NHS waiting lists are notoriously long, and GPs may be reluctant to refer patients on to specialist services.

Requesting your GP to refer you to Right to Choose is one way to avoid this (see psychiatry - UK and ADHD 360). This means you can select an organization that offers a private assessment and titration service. This can reduce your wait time by a significant amount and is more convenient if you are working and have a hectic schedule.

Once you have been identified as a candidate for this treatment the doctor you choose will assess you and decide whether or not you need to be titrated on medication. You will need to complete forms that are different for each provider however the most commonly used ones are self-reporting questionnaires and medical histories that go to childhood and adulthood. These will help your clinician determine any issues that could suggest you should not take ADHD medications if they are prescribed.

If you are being titrated on medication, this will usually consist of non-stimulant and stimulant medications. The combination will be carefully tested and monitored to find the most suitable combination for you, taking into account any current conditions you suffer from. The titration process can be a lengthy process and may require a number of additional appointments before your dose is stable.

The frequency of your follow-up appointments will depend on the type and quantity of medication that you are taking. They could be scheduled monthly or quarterly. The appointment will involve your doctor making sure that you are taking the correct medication, recording any side effects you've experienced, and making any necessary adjustments to ensure that you remain on a dose that is safe. You are required to attend an annual review and a routine health exam that includes pulse, blood pressure and weight measurements.

Some people may be satisfied with their medication and do not require discussing their titration in more detail. However, for those who require further discussions regarding their titration and treatment plan it's possible that they will be passed back to the same private clinic that conducted their initial assessment. If this occurs, the titration consultation will be part of their consultation fees and they will be given their medication via private prescription (see Consultation fees above). This can cost more than a prescription from the NHS, as the pharmacy will charge you for the medication.
